Listed below are items needed for registration.
THE FOLLOWING INFORMATION MUST BE COMPLETED BEFORE YOU CHILD CAN ENTER SCHOOL AND BE ASSIGNED A CLASS:
1. Birth Certificate (Original) — If kindergarten, the child must turn five (5) years of age on or before September 30, 2009. No child born after this date will be admitted. If you do not have the child’s birth certificate, you will need to fill out a Birth Certificate Affidavit form and have it notarized.
2. Social Security Card — copies are not accepted.
3. Physical Form — Students must have proof of physical examination performed within one year of entrance and before school begins. Physicals must be signed and dated by a physician.
4. Immunizations — Students must have these completed:
a. 3 DTP’s — 3rd after child’s fourth birthday
b. 3 OPV’s — 3rd after child’s fourth birthday
c. 2 MMR’s — 1st given at 10 months or older – 2nd dose anytime as long as one month has elapsed since the first dose
d. 3 Hepatitis B – for children born after January 1, 1994 at least 4 months spacing between 1st and 3rd doses (Varicella/chickenpox – not earlier than 12 months born after January 1, 1997)
5. 3 Proofs of Residence — A deed if buying, lease if renting or fill out a Resident Affidavit. Two other forms to present can be utility bill, driver’s license and/or any bill with your name and current address.
